Ugly spot on leg

Question:

Dear Tweety, I too had this terrible sore on my leg, just like you describe! I went to the drs. and he told me he thought it was a deer tic bite, he said a spider bite is much more uniform in shape. As I had just been on a hike in the woods a few days before, and did find a tic a few days later, dug in,but the wound diden’t look like the other one.My dr started me on doxicyline(sp.)and was treating me for lyme disease.I asked about UC causing this he said it could if I was having active disease which I’m not. After 2 days of treatment the sore started going away.As for a connection to Lyme Disease and IBD I’ve heard this quite often in the last few days. All I know was I was treated for Lyme Disease In 99, and was dx in 2000 with UC. I don’t know what bearing if any that has.Good luck to you!I hope you are better soon! Debbie

Response:

I read an article some time back where a doctor treated someone who had Crohns I believe but the doc discoverd she had lyme disease and gave her antibiotics and it supposedly cleared up. It’s interesting because whether there is a connection or not the treatment may just be the same. – Jeff – Hide quoted text — Show quoted text – > Dear Tweety, I too had this terrible sore on my leg, just like you describe! TweetyBird,  I have had this very nasty problem (Pyoderma Gangrenosum). My suggestion to you is to seek a very good skin Dr. or several.  I was treated for this from the onset with Prednisone. I dont mean to scare you but the Prednisone didnt do a thing for it. In fact I believe it only made it worse because Pred. is famous for poor wound healing. Mine was located apx. 6in. up from my left ankle on on the front side. It never got better,  6 months after the spot started it had grown to the size of a tennis ball. I had to be rushed into emergency because it had turned into full blown gangrene + I developed a staph infection. Again, Im not here to scare you but I remained in the hospital for 2 months. When I went into the ER they said that it would have to be debrieded (scraped) because the tissue had died. They said Id be out in 24 hrs. I ended up having 5 debriedments and lost a very healthy chunk of my leg….looks like a shark took a bite out of me with a scar the size of 6×5. They had to keep taking flesh because the tissue kept dying off. I was put on the strongest antibiotic they have, Minocycline I believe it was, plus more pred. via IV. The medical community seemed to be at a loss as to how to handle this, all of the nurses kept asking me if I was diabetic, they really dont have alot of experience with this. The part that really makes me furious is that after the last debriedment(chunk) of my leg was taken off they decided to do a skin graph to make it look better. They took skin from my tummy and put it on there, but what they didnt expect was that the same underlying promlem(Crohns) was going to eat up the graph 2 weeks after they did it. Please dont let this get any worse, If I knew then what I know now, I would have sought several skin Drs. I thought my gastro knew what he was dealing with but I was wrong and so was he, and he is the best around here. P.S. I live in MI. and went to the U of M hospital while it was still small and they sent me home saying that they,ve seen much worse, and told me that my doc was doing all that could be done. Good Luck Tweety,my thoughts are with you. If you have any questions you can E-mail me on the side. Best wish’s to you. Sallie

Response:

you certainly had a bad case.  i also live in michigan, and even though my ulcers have been in "remission" for 2 years, i continue to have autoimmunity issues.  what doc., what dept. did you go to at U of M?  i have an appointment in the rheumatology dept next month, i heard they are good, i hope this is not the dept that told you to just "go home"!! jeff, north street, mi, usa

I do not know that it has been established that Lyme patients have a higher % that other illnesses, just that many Lyme patients do develop this condition. There is very little research for Lyme despite it being the 2nd growing infectious disease in the US (2nd only to HIV). The Crohn’s Diseaes link is being looked at in the same way as some Lyme patients also have ALS (Lou G. Disease). I am one of those unfortunate ones misdiagnosed with a spider bite. My Dr.’s ignorance (and mine) have cost me a great deal. Kara Tyson Lyme D. Support Group Of AL

Response:

Is that a fact that people with lyme develop crohn’s. I have never seen that at all. or heard of such a thing?

Without starting some sort of war, could you please tell me where you got your information on Lyme Disease and Crohns Disease connections?  This is the first I have ever heard of such a connection and now I am curious.  Thanks in advance.   Margie CD Class of 67 UC Class of 96

Response:

i wish you well with it.  if you get the cause treated, your ulcers will be helped.  i have not had a recurrence in 2 years (knock on my head).  mine lasted about 6 months.  plan on wearing tall socks, they leave ugly scars. treat well, use whirlpool, the extent of the scaring can be reduced if you pay close attention to it now.  i know, it gets really, really tiring! hang in there… jeff, north street, mi, usa

Response:

My understanding is that if things like that don’t heal pretty well, they will put you on an IV antibiotic, which will take care of the problem quickly in most cases. Boyd — "The cure for boredom is curiosity. There is no cure for curiosity." (Ellen Parr- author)

YES!! I have had it, it is called superficial pyoderma gangrenosum.  The beauty of having an IBD problem.  Usually refuses to heal.  Doc used special zinc wrap (dressing) and whirlpool bath is good to keep the ulcer germ-free and rid it of any dead material clinging to its surface.  Cortisione is usually used, sometimes injected directly into the skin if you need something that agressive.  Tacrolimus and cyclosporine are also used to treat underlying causes (a rebellious immune system).  Believe it or not, thalidomide shows some promise for this condition.  Mine finally healed after using a flesh eating enzime on the sore for about 4 months! jeff, north street, mi, usa

It’s called pyoderma gangrenosum. I’ve never had any, but I’ve heard (in case your doc didn’t tell you this) that the ulcer should be kept padded and protected as much as possible. Any kind of rubbing, even washing, will increase the skin damage and later scarring.
